When to worry about a seizure with meningitis

Meningitis causes inflammation around the brain and spinal cord. That irritation can disturb brain activity and trigger a seizure. Fever, changes in brain pressure, infection-related complications, or an imbalance in the body can also contribute. A person may stiffen, jerk, lose awareness, stare blankly, or become confused afterwards. A first seizure cannot be safely explained at home, even when meningitis has already been diagnosed.

Keep the person on a safe surface and move nearby hard or sharp objects away. Turn them gently onto their side if possible, loosen tight clothing around the neck, and protect the head with something soft. Do not restrain them, put anything in the mouth, or give food, drink, or medicine until they are fully alert. Note when the seizure starts and describe what happened to the medical team.

Call emergency services immediately for a first seizure, a seizure that continues, repeated seizures without normal recovery, trouble breathing, a serious injury, blue or grey lips, or severe drowsiness that persists. Urgent care is also needed if the person develops worsening headache, repeated vomiting, a stiff neck, a new rash, weakness, or increasing confusion. Do not drive the person yourself if they are actively seizing or difficult to wake.

Ask what may have triggered the seizure, whether brain imaging or other tests are needed, how prescribed medicines should be given, and which warning signs require an immediate return to hospital. Ask whether someone should supervise the person at home and when normal activities can safely resume.
