When to worry about pimples with perioral dermatitis

Perioral dermatitis can cause small red or skin-coloured bumps around the mouth, nose, or eyes, sometimes with burning, dryness, or scaling. The skin may look acne-like without being typical acne. Triggers can include steroid creams on the face, heavy cosmetics, fragranced products, or an irritating skincare routine. A clinician can distinguish this pattern from acne, folliculitis, contact irritation, or infection.

Keep the routine simple: wash gently with lukewarm water or a mild cleanser, avoid scrubbing and picking, and pause new or fragranced products. Do not start or restart a steroid cream on the face unless the prescriber directs you. Tell your clinician about every cream, inhaler, or nasal spray that may contact your skin; treatment choices may need to account for these exposures.

Arrange a dermatology or primary-care review if the rash keeps returning, extends beyond the original area, forms crusts or pus, or is interfering with sleep or daily activities. Seek same-day advice for rapidly increasing swelling, marked tenderness, warmth, or fever. Get urgent help if the eyelids or eyes become painful, swollen, light-sensitive, or affected in vision.
