When to Worry About Pelvic Pain With Ovarian Cysts

An ovarian cyst may cause one-sided pelvic aching, pressure, bloating, or pain during movement. Some cysts cause no symptoms. Pain can also change if a cyst bleeds, ruptures, or affects the ovary's position, so a familiar diagnosis does not explain every new symptom.

Record the side, timing, severity, relation to movement or periods, and associated bleeding, nausea, fever, or urinary symptoms. This information helps a clinician decide whether observation, imaging, or another evaluation is appropriate.

For mild, familiar discomfort, rest and use only pain relief that is safe for you and recommended by a clinician or pharmacist. Avoid strenuous activity if it clearly increases the pain. Arrange a review when symptoms persist, recur, or disrupt normal activities.

Do not rely on an earlier scan if the pain has changed. A clinician may need to reassess you and consider a pregnancy test, examination, or ultrasound depending on your symptoms.

Go to urgent care for sudden intense pelvic pain, pain with repeated vomiting, fainting or marked weakness, a swollen or rigid abdomen, fever, or heavy vaginal bleeding. Seek urgent assessment as well if you could be pregnant, even if you have been told you have a cyst.

For moderate or ongoing pain without these warning signs, contact your clinician promptly rather than waiting for the next scheduled appointment.
