When to Worry About Painful Urination With a Urinary Tract Infection

A urinary tract infection can inflame the bladder or urinary passage, leading to burning, urgency, frequent small amounts of urine, lower abdominal discomfort, or cloudy and strong-smelling urine. Fever, pain in the side or back, nausea, or shaking chills may suggest that infection is affecting the kidneys. Symptoms can overlap with irritation, stones, or sexually transmitted infections, so an assessment may be needed.

Contact a clinician if symptoms are new, do not settle, return after treatment, or occur during pregnancy, in a child, or in someone with a urinary catheter or weakened immune system. Drink normally unless fluid restriction has been advised. Follow prescribed treatment, and do not use another person’s antibiotics. Note when symptoms started, whether urine is bloody, and whether pain is moving toward the back.

Seek urgent medical care for fever with chills, pain in the flank or back, repeated vomiting, marked weakness, confusion, fainting, or inability to pass urine. A person who is pregnant and has urinary symptoms should seek prompt clinical advice. Visible blood, severe pain, or symptoms that worsen despite treatment also need timely review.
