When to worry about painful periods with uterine fibroids

Fibroids can change the shape of the uterus and interfere with normal contractions, causing stronger cramps or pressure in the pelvis. They may also make periods longer or heavier. Pressure can be felt in the lower abdomen or back, and a larger fibroid may cause frequent urination or bowel pressure. Painful periods can have several explanations, so the symptom does not by itself confirm that fibroids are responsible.

Book a review if cramps are stopping normal activities, pain relief no longer helps, bleeding is becoming heavier, or symptoms are affecting sleep. Keep a record of bleeding days, pad or tampon changes, clots, pain location, and dizziness. A clinician may discuss an examination, blood tests, or imaging depending on your symptoms and previous diagnosis. Do not start or change hormonal or anti-inflammatory treatment without checking that it suits you.

Get urgent help for bleeding that feels difficult to control, severe pain that arrives suddenly, fainting, chest discomfort, breathlessness, or profound weakness. Prompt assessment is also needed for new pelvic pain during pregnancy or after menopause. These features can reflect blood loss or another urgent pelvic problem and should not be managed at home alone.

Ask whether your fibroid location could explain the pain or bleeding, whether you need testing for anaemia, and which treatment choices fit your plans for pregnancy. Ask what change in bleeding should trigger urgent review.
