When should you worry?

Arrange an appointment if pain stops you from working, studying, sleeping, or carrying out usual activities; does not improve with your treatment plan; returns between periods; or comes with pain during sex, bowel movements, or urination. Sudden severe pain, fainting, heavy bleeding, fever, vomiting, or a possible pregnancy needs prompt assessment because another problem may be present.

What the change in pain may mean

Endometriosis can cause inflammation and tender areas around the pelvis. Pain may become more intrusive over time, follow a different pattern, or affect nearby organs. The intensity of pain does not reliably show how much endometriosis is present, so a change in symptoms is worth discussing even when earlier examinations were reassuring.

Pain can also have causes unrelated to endometriosis, including infection, an ovarian cyst, or pregnancy-related conditions. A clinician may ask about timing, bleeding, bowel and bladder symptoms, medicines, and whether pregnancy is possible before deciding what assessment is appropriate.

What to do before your appointment

Keep a brief symptom record: when pain starts, where it is felt, how it affects daily activities, bleeding changes, and what treatments help or fail. Bring a list of medicines and note any side effects. Continue only the medicines and self-care measures already advised for you; do not increase doses or combine pain medicines without checking with a pharmacist or clinician.

If symptoms interfere with work, relationships, sleep, or plans, say this plainly. Functional impact helps the clinician judge whether your current approach needs review and whether further examination or referral could help.

When to seek urgent care

Seek emergency help for sudden or unbearable pelvic pain, collapse or fainting, heavy bleeding with weakness or dizziness, trouble breathing, or severe pain when pregnancy is possible. Seek same-day medical advice for severe pain with fever, repeated vomiting, or feeling acutely unwell.

If the pain is concerning but not an emergency, contact a gynaecology service or primary-care clinician soon. Take your symptom record with you and mention whether the pain is new, escalating, or different from your usual endometriosis symptoms.
