When to worry about painful breastfeeding with nipple thrush

Nipple thrush may cause burning, stinging or deep breast pain during and after feeds. Nipples can look shiny, flaky, cracked or unusually red, although appearance alone cannot confirm the cause. A baby may have white patches in the mouth, but either parent or baby can have symptoms without the other showing obvious signs. Similar pain can also result from shallow attachment, skin irritation, eczema or bacterial infection, so a clinician should consider the whole picture.

Keep nipples clean and dry, change breast pads when damp, and wash hands before and after feeds or applying treatment. Check your baby’s latch and avoid fragranced products that may irritate the skin. Continue feeding if you can; expressing gently may be an option when direct feeding is too uncomfortable. Do not use leftover antifungal medicine or treat only one of you without professional guidance, because parent and baby may need coordinated care.

Arrange medical advice if pain continues after feeds, interferes with feeding, returns after treatment, or is accompanied by cracked skin that is not healing. Seek prompt care for fever, a hot red area, rapidly increasing swelling, pus or feeling very unwell, as these features can point to another breast problem. Get urgent help for fainting, confusion, breathing difficulty or severe dehydration. Your baby should be assessed promptly for poor feeding, fewer wet nappies or unusual sleepiness.
