When to worry about pain during sex with ovarian endometrioma

An ovarian endometrioma is a cyst associated with endometriosis. A deep ache or sharp pain during penetration may occur when the ovary or surrounding pelvic tissues are moved or pressed. Pain can continue after sex, occur around menstruation, or accompany painful periods, pelvic aching, or difficulty becoming pregnant. The pattern alone cannot confirm the cause, because infections, pelvic floor tension, and other pelvic conditions can also cause pain.

Stop or change sexual activity when pain starts; do not push through it. Note where the pain occurs, whether penetration is deep or at the entrance, its relation to your cycle, and any bleeding or urinary symptoms. A clinician may review symptoms, examine you, and consider pelvic imaging. Until reviewed, gentler positions, slower movement, and lubricant may reduce friction, but persistent or recurrent pain still deserves assessment.

Contact a healthcare professional soon if pain repeatedly returns, limits sex, or comes with increasing pelvic pressure, unusual discharge, bleeding after sex, or changes in your periods. Go to urgent care for abrupt intense pain, collapse or marked dizziness, fever with pelvic pain, or vomiting that prevents fluids. These symptoms can signal a complication or another condition requiring prompt evaluation.
