When to worry about neck stiffness with meningitis

Neck Stiffness is a very common presentation of Meningitis. In meningitis, inflammation around the brain and spinal cord can make moving the neck painful or difficult. The symptom may occur with a strong headache, fever, nausea, sensitivity to light, or a change in alertness. A rash that does not fade when pressed can also be a warning sign, but meningitis does not always cause every symptom. Neck stiffness by itself has many possible causes, so it cannot confirm the diagnosis.

If meningitis is possible, arrange emergency medical help immediately rather than waiting for the stiffness to settle. Do not drive yourself if you feel confused, faint, very drowsy, or unwell. Tell the clinician when the stiffness began, whether there is fever or headache, and whether you have taken any medicine. Keep the person safe, rested, and accompanied while help is arranged. Do not delay care to check whether a rash appears.

Call emergency services for neck stiffness with a severe or rapidly worsening headache, high fever, confusion, a seizure, difficulty waking, breathing problems, weakness, repeated vomiting, or a new non-fading rash. Babies and older adults may show less typical signs, such as poor feeding, irritability, marked sleepiness, or reduced responsiveness. Even without a rash, a combination of fever, headache, and neck stiffness warrants urgent assessment. A clinician may need to examine you promptly and arrange tests or treatment.
