When to worry about neck pain with whiplash

Whiplash is a forceful back-and-forth movement of the neck, often during a road collision or similar event. It can irritate muscles, ligaments, joints, and nearby tissues, leading to neck pain, stiffness, reduced movement, shoulder discomfort, or headache. Symptoms can change after the event. New problems with arm strength, sensation, balance, alertness, or vision should not be assumed to be ordinary muscle soreness.

If a clinician has assessed you, follow the agreed plan and keep gentle movement within comfort. Avoid sudden loading, contact activity, or driving if neck movement is too restricted for safe observation. Use prescribed or pharmacist-advised pain relief as directed. Write down the injury mechanism, symptom progression, arm sensations, and any headache or dizziness to make the review more useful.

Get emergency help for neck pain after a major impact with weakness, loss of feeling, trouble walking, fainting, confusion, severe headache, repeated vomiting, or loss of bladder or bowel control. Seek prompt review when pain is worsening, movement is becoming more limited, an arm becomes clumsy, or symptoms are not starting to improve. A clinician may need to check the spine and nervous system.

Ask whether your examination suggests soft-tissue whiplash or another injury, which activities are safe, and how much movement to practise. Ask what symptoms require emergency care and when a follow-up assessment should occur if recovery is slow.
