When to worry about neck pain with a herniated disc

A herniated disc in the neck can irritate a nearby nerve. Pain may spread from the neck into the shoulder, arm, or hand, sometimes with pins and needles, numbness, or weakness. Symptoms may change with neck position or activities that load the arm.

The same symptom pattern can have other causes. Examination of strength, reflexes, sensation, and movement helps a clinician judge whether a nerve is irritated and whether the spinal cord may also be affected.

Seek a routine review if pain persists, limits normal activity, or travels into the arm. Keep moving gently within a comfortable range, take posture breaks, and avoid heavy lifting or repeated neck bending that increases arm symptoms. Do not attempt forceful neck manipulation.

Note the location of pain, numbness, and weakness, along with changes in grip or dexterity. Bring a list of medicines and describe any injury or sudden exertion that preceded the symptoms.

Get urgent medical help for rapidly increasing weakness, inability to use an arm, severe or spreading numbness, new clumsy hands, walking difficulty, or loss of bladder or bowel control. Neck pain after major trauma with neurological symptoms needs immediate assessment. Sudden facial drooping, speech difficulty, or one-sided weakness is an emergency.
