When to worry about thickened nails with nail psoriasis

Psoriasis can affect the nail matrix and nail bed, producing thickening, ridges, pits, colour change, or separation from the skin beneath. Thickened Nails is a common presentation of Nail Psoriasis. A nail can also have infection or injury at the same time, so a new pattern should not automatically be attributed to psoriasis.

Trim carefully after softening the nail, keep the edge smooth, and moisturise the nail and surrounding skin. Avoid picking at lifted areas, biting, harsh cuticle work, and repeated trauma. Follow the treatment plan prescribed for your psoriasis and mention any new nail change at your next review.

Arrange assessment if the nail becomes painful, separates further, develops pus or spreading redness, or interferes with walking or using your hands. Seek advice when one nail changes rapidly, a dark mark appears without injury, or the nail does not behave like your usual psoriasis. These features may need examination for another process.
