When should you worry about itching worse at night if you have scabies?

Scabies mites burrow into the outer layer of skin and provoke an immune reaction. Itching often becomes more noticeable at night. A rash may include small bumps, scratch marks, or thin burrows, commonly around the finger webs, wrists, waist, armpits, or genitals. A clinician may need to distinguish scabies from eczema, bites, or another itchy rash.

Use the prescribed or recommended scabies treatment exactly as directed, including the areas and timing specified. Close household or intimate contacts may need assessment at the same time, even if they have no itch. Wash or isolate recently used clothing, towels, and bedding as advised. Keep nails short and use gentle skin care to reduce damage from scratching. Itch can linger after mites have been cleared, so do not automatically repeat treatment.

Contact a clinician if the rash is widespread, the diagnosis is uncertain, or symptoms remain active after the recommended plan. Seek prompt care for increasing warmth, pain, pus, honey-coloured crusts, fever, or rapidly spreading redness. People with weakened immunity, very young children, and older adults should be assessed early when scabies is suspected. Facial swelling or breathing difficulty after a medicine needs urgent help.
