When to worry about a hoarse voice with throat cancer

Throat cancer can affect structures involved in voice production, while surgery, radiotherapy, swelling, infection, or reflux may also alter the sound. Hoarseness may be rough, weak, breathy, or tiring. The symptom alone cannot determine the cause or the extent of disease, so its timing and accompanying changes matter.

Tell your specialist when the hoarseness started, whether it is changing, and whether speaking or swallowing has become harder. Avoid shouting, smoke, and frequent throat clearing; sip fluids if swallowing is safe. Follow your treatment plan and do not stop medicines or therapy because of a voice change without advice.

Arrange prompt review for hoarseness that persists or worsens, a new neck lump, pain when swallowing, food sticking, unexplained weight loss, ear pain, or coughing blood. Seek urgent help for noisy breathing, rapidly worsening swelling, severe breathlessness, or inability to swallow saliva.

Ask whether the change may come from the tumour, treatment, infection, or reflux, and whether an ear, nose, and throat examination is needed. Ask about voice therapy, safe exercises, and the symptoms that should lead to an urgent call.
