How gluteal tendinopathy behaves

Gluteal tendinopathy involves the tendons that attach the buttock muscles to the outside of the hip. Pain is often felt over the outer hip and may flare with lying on that side, stairs, running, or standing on one leg. Tenderness and reduced confidence on the affected side can occur. A flare after a sudden increase in walking, hills, or exercise does not automatically mean the tendon has torn, but persistent loss of function warrants a review.

Practical steps

Temporarily reduce the movements that sharply increase pain, especially repeated stairs, steep hills, and side-lying on the painful hip. Keep gently moving within a tolerable range rather than staying still for long periods. A physiotherapist can adjust strengthening so the tendon is loaded gradually. Arrange an appointment if pain continues to limit sleep, work, or walking, or if your current programme repeatedly causes a flare.

Warning signs

Get urgent help after a fall or direct blow if you cannot bear weight, or if pain is sudden and severe. Seek prompt assessment for new pronounced weakness, a hot swollen joint, fever, spreading redness, or numbness in the leg. Gradual outer-hip pain without these features is usually suitable for a planned clinical review, where other causes of hip pain can also be considered.
