Older adults and people with heart, lung, immune, or other long-term conditions may become seriously unwell with less obvious fever. A new or worsening cough, chest pain with breathing, or reduced fluid intake deserves medical advice.

Practical steps at home

Take prescribed antibiotics exactly as directed and do not use leftover medicines. Rest, drink regularly, and keep a written note of temperature, breathing changes, medicines, and fluid intake. Sit upright if that makes breathing easier, and avoid smoke and strenuous activity while recovering.

Arrange review if fever persists, returns after improvement, or symptoms are not moving in the expected direction. A clinician may need to check oxygen levels, listen to the lungs, or reconsider the treatment plan.

When to seek care

Call emergency services for severe difficulty breathing, blue or grey lips, new confusion, fainting, collapse, or inability to stay awake. Seek same-day medical care for increasing breathlessness, chest pain, repeated vomiting, dehydration, or a fever in someone who is frail, pregnant, very young, or immunocompromised. Do not drive yourself if you feel faint or confused.
