When to worry about facial pain and pressure with chronic sinusitis

Chronic sinusitis can keep the lining of the sinuses inflamed, making drainage difficult. This may cause pressure around the cheeks, forehead, nose, or upper teeth, often alongside blocked breathing or reduced smell. Symptoms may fluctuate rather than follow a single brief illness. Pain that is new, strongly one-sided, or different from your usual pattern deserves a fresh assessment because facial pressure has other possible causes.

Record where the pressure is, when it appears, and whether congestion, nasal discharge, reduced smell, dental pain, or headache occurs with it. Follow your clinician’s treatment plan and use prescribed nasal medicines as directed. Gentle saline rinsing may help some people when performed with safe, clean water and correctly cleaned equipment. Avoid repeatedly starting leftover antibiotics or using decongestant sprays longer than advised.

Book a review if pressure persists despite your plan, keeps returning, interferes with sleep, or is accompanied by persistent one-sided blockage or nosebleeds. Seek urgent care for swelling around an eye, vision changes, severe headache unlike your usual pain, confusion, neck stiffness, weakness, or high fever with a rapidly worsening illness. Dental pain or tenderness may need dental assessment as well as sinus evaluation.
