When to worry about coughing up phlegm with chronic sinusitis

Inflamed sinuses can drain mucus backwards into the throat, causing throat-clearing, a cough, or phlegm that is noticed after waking or when lying down. Nasal blockage, facial pressure, reduced smell, and an unpleasant taste may occur too. Phlegm from the chest, wheezing, or shortness of breath may suggest an additional airway problem.

Use saline nasal rinses only with sterile, distilled, or previously boiled and cooled water, and clean the device as directed. Follow prescribed nasal treatment consistently. Avoid smoke and irritants, and arrange a review if symptoms keep returning, interfere with sleep, or do not respond to the plan.

Seek prompt medical advice for blood in phlegm, fever, worsening facial swelling or pain, severe headache, vision changes, neck stiffness, or new chest symptoms. Get emergency help for severe breathing difficulty, confusion, fainting, or swelling around the eye with vision loss.

Ask whether the mucus is likely from postnasal drainage or the lower airways, whether your nasal treatment technique is correct, and whether allergies or asthma could be contributing. Ask which eye, breathing, or neurological symptoms require immediate care.
