When to worry about coughing up blood with pulmonary embolism

A pulmonary embolism is a clot affecting blood flow in the lungs. It can irritate lung tissue and cause blood-streaked mucus. The amount may be small, but the symptom matters more when it occurs with sudden breathlessness, sharp chest pain, a fast heartbeat, dizziness, or pain and swelling in one leg.

Contact your treating clinician promptly about any coughing up blood, even if the streaks are limited. Take prescribed anticoagulant medicine exactly as directed and do not change or stop it without medical advice. Note the colour, amount, timing, and whether breathlessness or chest pain is changing.

Call emergency services immediately for heavy or repeated bleeding, severe or sudden breathlessness, chest pain, fainting, confusion, blue lips, or a very fast or irregular heartbeat. Do not drive yourself if you feel faint or short of breath. Keep medication details available for the emergency team.

Ask whether the bleeding fits your known clot, whether your anticoagulant plan needs review, and which changes in breathing or bleeding should trigger emergency care. Ask how to record symptoms and when follow-up imaging or blood tests may be needed.
