When to worry about cough with heart failure

Cough is a common presentation of Heart Failure. Reduced pumping can allow fluid to build up in or around the lungs, causing cough, breathlessness, or difficulty lying flat. A new cough may also have another cause, so its timing and accompanying symptoms help guide assessment.

Take heart-failure medicines exactly as prescribed and do not change a diuretic dose without clinical guidance. Note whether the cough is worse at night, whether you need extra pillows, and whether swelling or breathlessness is increasing. Limit exertion while symptoms are changing and arrange a review.

Contact your heart-failure team promptly for a new or worsening cough, increasing ankle or abdominal swelling, rapid change in exercise tolerance, or breathlessness when lying down. Call emergency services for severe breathing difficulty, chest pain, fainting, marked confusion, or coughing pink and frothy mucus.

Ask whether fluid congestion could explain the cough, whether your treatment needs review, and which symptoms should prompt same-day or emergency care.
