When to worry about cough with COPD

Cough is a very common presentation of COPD. It may be dry or bring up mucus as the airways stay inflamed and narrowed. Your baseline matters: a noticeable change in sputum amount or colour, more wheezing, worsening breathlessness, fever, or reduced ability to walk can indicate a flare or infection. A cough can also have causes unrelated to COPD, particularly when it is new, painful, or accompanied by blood.

Follow your COPD action plan and use prescribed inhalers with your usual technique. Stop smoking and avoid fumes, dust, and other known triggers. Pace activity, rest between tasks, and keep hydrated unless fluid restriction has been advised. Note changes in sputum, temperature, breathing, and reliever use so your clinician can judge the difference from baseline. Do not start leftover antibiotics or steroids without advice.

Call emergency services for severe or rapidly worsening breathlessness, blue or grey lips, confusion, fainting, or chest pain that may be serious. Seek same-day advice for increased sputum, a colour change, fever, new blood, or a reliever that is not controlling symptoms as your plan expects. Book a review when cough steadily worsens, sleep is affected, or everyday activity becomes harder.

Ask how your usual sputum and breathlessness compare with a flare, when to begin each action-plan step, and whether inhaler technique should be checked. Discuss pulmonary rehabilitation, smoking support, vaccination advice, and the symptoms that require emergency care.
