When to worry about cough with bronchiolitis

Cough is a very common presentation of Bronchiolitis. In infants and young children, swelling and mucus in the small airways can make breathing noisy and feeding tiring. Cough may come with a runny nose, faster breathing, wheeze, or reduced appetite. The cough itself matters less than the child’s breathing effort, hydration, responsiveness, and ability to feed.

Keep the child upright while awake and offer smaller, more frequent feeds if this is easier. Clear nasal mucus gently before feeds when advised, and monitor wet nappies or urination, alertness, and breathing at rest. Avoid smoke exposure and do not give cough medicines to a young child unless a clinician recommends them. Check the child regularly, including during sleep, when symptoms are concerning.

Call emergency services if the child stops breathing, has marked pulling in at the ribs or neck, turns blue or grey, is difficult to wake, or cannot take fluids. Seek same-day medical advice for fewer wet nappies, repeated vomiting, worsening cough with faster breathing, or feeds that are much shorter than usual. Babies born early or with heart, lung, or immune problems need a lower threshold for assessment.

Ask how to count breathing when the child is calm, which feeding changes matter, and what hydration signs to watch. Confirm who to call after hours and whether the child’s age or medical history changes the safety plan.
