When to worry about cold hands and feet with hypothyroidism

Hypothyroidism can slow the body’s processes that generate and distribute heat, so fingers and toes may feel cool even in a comfortable room. This symptom can occur with tiredness, dry skin, constipation, slowed thinking, or changes in weight. Coldness alone cannot show whether thyroid hormone levels are outside your treatment target, and it can have other causes such as circulation problems, anaemia, or exposure to cold.

Keep a short record of when the coldness happens, whether both sides are affected, and any colour change or numbness. Wear warm socks and gloves, avoid direct heat against numb skin, and take prescribed thyroid medicine exactly as directed. Arrange a routine review if the symptom persists, returns after a treatment change, or comes with increasing fatigue. A clinician may assess your circulation and review thyroid blood tests and other possible causes.

Get urgent medical help if one hand or foot suddenly becomes cold, pale, blue, very painful, numb, weak, or difficult to move. Seek emergency care for coldness with chest pain, severe breathlessness, fainting, confusion, or marked drowsiness. These features need prompt assessment and should not be explained away as thyroid symptoms.

Ask whether your symptoms and examination suggest a thyroid-related cause, whether your thyroid tests should be repeated, and whether another blood test or circulation assessment is appropriate. Tell the clinician about missed doses, recent medicine changes, new numbness, skin colour changes, and any conditions or medicines that affect blood flow.
