When to worry about chest tightness with COPD

COPD can make airflow harder, leaving the chest feeling tight or full, particularly during activity or a flare. Increased cough, wheeze, phlegm, or a change in phlegm can accompany the sensation. A new tight feeling may also reflect infection, heart problems, or another condition, so compare it with your usual pattern and report meaningful changes to a clinician.

Pause and sit upright, loosen tight clothing, and use your prescribed inhalers according to your COPD plan. Practise pursed-lip breathing if this is part of the technique taught to you. Avoid smoke and exertion while symptoms settle. Do not add extra doses or antibiotics unless your clinician has given clear instructions for that situation.

Get emergency help for severe or rapidly worsening breathlessness, blue or grey lips, new confusion, fainting, inability to speak comfortably, or chest tightness with crushing pain, sweating, or nausea. Arrange urgent assessment when your usual inhaler is not helping, you cannot manage normal activities, or fever and sputum changes accompany a flare. Keep your medication list available.

Ask how your COPD action plan defines a flare and exactly when to call for help. Discuss inhaler technique, breathing exercises, signs of infection, oxygen advice if prescribed, and how to distinguish your usual tightness from a new chest problem.
