When to Worry About Bone Pain With a Stress Fracture

Short answer
Persistent Bone Pain is a common presentation of Stress Fracture. Arrange medical assessment when pain persists, worsens with activity, occurs at rest, or limits walking, because examination and imaging may be needed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical trigger
- Repeated loading or a sudden increase in activity
- Useful observation
- Pain during activity that persists or appears at rest
- Assessment
- Examination with imaging when appropriate
What the pain may mean
A stress fracture is a small crack caused by repeated loading rather than one major injury. Pain may begin during exercise and later appear during ordinary movement or rest. Local tenderness and swelling can occur. Persistent pain can also have other causes, so the symptom alone cannot identify the cause. An examination may help locate the painful bone and assess movement, strength, and tenderness.
What to do now
Stop the activity that brings on pain and avoid running, jumping, or carrying loads until you have advice. Do not test the area repeatedly. Use support or reduced weight-bearing if a clinician recommends it. Keep a record of when pain occurs, what makes it worse, any swelling, and whether there was a recent change in training, footwear, or activity. Imaging may be arranged if the examination suggests a fracture.
When to seek care
Seek prompt medical care for pain that is severe, rapidly worsening, present without activity, or accompanied by marked swelling, deformity, numbness, or inability to bear weight. After a known stress fracture, new pain in another area or pain that is not improving with the advised plan also deserves review. Sudden severe pain after an injury should be assessed urgently.
Questions for your doctor
Ask which activities and weight-bearing level are safe, whether imaging is needed, how to protect the bone while it heals, and which symptoms should prompt an earlier review. Ask when and how to return to exercise without provoking pain.
Questions people ask
Can I exercise with suspected stress-fracture pain?
Avoid the activity that causes pain and seek clinical advice before returning to impact exercise.
Does every painful bone have a stress fracture?
No. Persistent bone pain has several possible causes, and an examination may be needed to distinguish them.
