When to worry about a blocked nose with chronic sinusitis

Chronic sinus inflammation can swell the lining inside the nose and narrow the drainage pathways. Mucus may become difficult to clear, causing pressure, reduced smell, mouth breathing, or a blocked feeling that comes and goes. Symptoms affecting both sides can fit this pattern, while persistent blockage on one side deserves an examination because structural narrowing, a polyp, or another nasal problem may also be involved.

Use any prescribed nasal treatment exactly as directed and allow the technique time to work. Saline rinsing may help clear thick mucus when performed with sterile or previously boiled and cooled water. Keep a note of whether the blockage is one-sided, whether smell changes, and what triggers flare-ups. Avoid smoke and other irritants. Do not keep using a decongestant spray beyond its labelled limit, because overuse can worsen congestion.

Arrange a clinical review when blockage persists despite the treatment plan, repeatedly disrupts sleep, or is paired with ongoing facial pain, fever, foul-smelling discharge, nosebleeds, or a marked loss of smell. Seek urgent help for swelling around an eye, vision changes, severe headache, confusion, a stiff neck, or difficulty breathing. These features need assessment rather than home treatment.

Ask whether the pattern suggests uncontrolled inflammation, a nasal polyp, or a structural obstruction; whether your nasal spray technique is correct; and whether an examination or specialist assessment is needed. Bring your symptom timeline and a list of medicines, including sprays bought without a prescription.
