What the pattern can suggest

Pain at the base of the big toe with a visible bump may occur with bunions. Sudden hot, red, swollen joint pain can occur with gout. Stiffness when the big toe bends may point to hallux rigidus, while a curled or crowded toe can be a hammer toe. Pain under the front of the foot after sports may involve the plantar plate, and a forceful bend of the big toe can cause turf toe. These patterns overlap, so the symptom does not establish one diagnosis.

What you can do now

Wear roomy shoes with a broad toe box and avoid activities that load the painful toe. Rest and elevate the foot after activity; a cool pack wrapped in cloth may ease discomfort for a short period. Do not cut into a corn, force a crooked toe straight, or continue sport through sharp pain. Note the affected toe, onset, footwear, injury, skin changes, and whether the joint is hot or swollen.

When it is urgent

Get urgent care after an injury if the toe points abnormally, is open through the skin, becomes pale or blue, or loses sensation. Seek prompt help for a hot swollen joint with fever, rapidly spreading redness, or a wound that is draining. Arrange an appointment for persistent pain, repeated swelling, worsening deformity, or difficulty fitting shoes or walking.
