What a limp can reflect

People may limp to reduce pain, protect an injured area, or compensate for weakness or altered leg length. Hip osteoarthritis and knee osteoarthritis can present with a gradually changing walking pattern. Stress fracture can present with activity-related pain and protective limping. In children and teenagers, Perthes disease or slipped capital femoral epiphysis can present with a limp and hip, thigh, or knee discomfort.

Steps before an appointment

Reduce activities that increase pain and use a handrail or suitable support if balance is uncertain. Wear stable footwear and avoid testing the painful leg through repeated impact. Note whether the limp began after a twist, fall, new exercise, or illness, and record pain location, swelling, fever, and whether the pattern changes during the day. Children with a new limp should be assessed rather than encouraged to walk it off.

Signs that need prompt assessment

Seek urgent care for inability to bear weight, a visibly shortened or abnormal limb, severe pain after injury, rapidly increasing swelling, or a hot joint with fever. A child who suddenly refuses to walk, has significant hip pain, or appears unwell needs prompt medical evaluation.
