When is vaginal pessary fitting recommended for pelvic organ prolapse?

Pelvic organ prolapse happens when one or more pelvic organs shift downward and press into the vaginal space. The affected organ may be the bladder, uterus, or bowel. A pessary is a removable device that sits inside the vagina and helps hold the tissues in a more supported position. It can reduce a bulge or dragging sensation and may help some bladder or bowel symptoms, although the response differs between people.

A clinician will consider the type and extent of prolapse, your symptoms, vaginal tissue health, activity, and whether you want to manage the device yourself. Fitting can involve trying different shapes and sizes. Once it feels comfortable, learn removal, cleaning, storage, and replacement instructions. Keep follow-up visits even if symptoms improve, because pressure from a poorly fitting device can irritate tissue.

Report persistent discomfort, bleeding, unusual or unpleasant discharge, difficulty passing urine or stool, or a pessary that repeatedly slips out. Seek urgent care for severe pelvic pain, heavy bleeding, fever, or inability to urinate. A new or rapidly worsening bulge also deserves clinical assessment rather than self-adjustment.
