When Is a Thyroid Function Test Recommended for Goitre?

Goitre describes enlargement, not the thyroid’s level of activity. Some enlarged glands produce excess hormone, some produce too little, and some continue to function normally. A blood test commonly measures thyroid-stimulating hormone and may include thyroid hormones when the result or symptoms call for more detail. The result helps guide the next step, but it does not by itself explain every lump or enlargement. A clinician may also consider an examination, ultrasound, medicine history, iodine exposure, and symptoms such as palpitations, weight change, tiredness, feeling unusually hot or cold, or difficulty swallowing.

Arrange an appointment if you notice neck swelling, a change in an existing goitre, or symptoms suggesting altered thyroid activity. Bring a list of medicines and supplements, especially products containing thyroid hormone or biotin, and mention pregnancy or plans for pregnancy. Do not stop prescribed treatment unless instructed. The clinician will explain whether repeat blood testing, imaging, or specialist review is appropriate after examining your neck and considering the result.

Seek urgent assessment if neck swelling causes difficulty breathing, noisy breathing, inability to swallow fluids, or rapidly worsening pressure. Prompt review is also appropriate for a new or quickly enlarging swelling, persistent hoarseness, or significant pain. These features do not establish a diagnosis, but they should not be left for a routine appointment.

Ask which thyroid markers will be tested, whether any supplement could affect the result, whether the goitre needs imaging, and when results should be reviewed. Ask which neck or hormone changes should prompt earlier contact.
