When is premarital screening recommended for sickle cell disease?

Sickle cell disease is inherited. A person may have the condition or carry a haemoglobin gene variant without having the same symptoms. Screening usually starts with a blood test that examines haemoglobin. If a result suggests a carrier state or disease, additional testing and counselling may clarify it. The result concerns both partners because their combined genetic information can affect the chance of a child inheriting a serious haemoglobin disorder.

Arrange screening early enough to allow time for results, confirmatory testing, and a genetics discussion before making decisions about marriage or pregnancy. Bring any previous blood-test reports and tell the clinician about relatives with sickle cell disease, unexplained anaemia, or other haemoglobin disorders. Each partner should be tested when advised. Genetic counselling should explain choices clearly and confidentially; a carrier result is not an illness and is not caused by anything either person has done.

Screening is planned care, but seek medical advice promptly for severe pain, breathing difficulty, fever, marked tiredness, or yellowing of the eyes in someone known to have sickle cell disease. These symptoms may need assessment separate from premarital testing. Ask for specialist support if a result is unclear, if both partners carry a relevant variant, or if either partner has symptoms or a known diagnosis.
