When is oral antifungal treatment recommended for scalp ringworm?

Scalp ringworm can cause scaling, itch, broken hairs, tender areas, or patches of hair loss. Because the fungus can enter hair shafts and follicles, a clinician generally assesses it promptly and chooses medicine that reaches those structures. An examination may be supported by hair or skin testing. A similar-looking scalp problem may have another cause, so avoid assuming that every flaky or thinning patch is fungal.

Book a medical assessment, especially for a child with a scaly patch or hair loss. Tell the clinician about allergies, liver conditions, pregnancy, and current medicines. Use the prescribed oral medicine for the full planned course and attend any requested follow-up. An antifungal shampoo may be added to reduce spread, but it does not replace the oral medicine. Do not share combs, hats, brushes, pillows, or towels; wash these items and ask whether household members or pets need assessment.

Seek prompt care for a very tender, swollen, soft area of the scalp, pus, fever, swollen neck glands, or rapidly expanding hair loss, as these features can signal a stronger inflammatory reaction and risk of scarring. Report facial swelling, breathing trouble, yellowing of the eyes, or a severe rash after starting treatment.
