When is loop excision of the cervix recommended for cervical dysplasia?

Short answer

Loop excision may be recommended when cervical dysplasia is high grade, persists, or needs removal and laboratory examination after colposcopy.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How the decision is made

Cervical dysplasia means abnormal cells on the cervix; it is not the same as invasive cervical cancer. Colposcopy and a biopsy help identify the grade and location of the changes. A loop excision uses a thin heated wire to remove the affected area, usually through the vagina. The specimen is examined to confirm the diagnosis and whether the abnormal area has been removed. Some low-grade changes can be monitored, while higher-grade or persistent changes more often lead to an excisional discussion.

Preparing for the procedure

Ask how your biopsy result, transformation-zone visibility, and follow-up history affect the recommendation. Tell the clinician if you may be pregnant, take blood thinners, or have unusual bleeding. Discuss pregnancy plans because removing cervical tissue can influence later pregnancy monitoring. After treatment, follow instructions about bleeding, discharge, intercourse, swimming, and tampons, and attend the recommended cervical follow-up even if you feel well.

When to contact a clinician

After loop excision, seek prompt advice for heavy bleeding, large clots, worsening pelvic pain, fever, or unpleasant-smelling discharge. Before treatment, urgent assessment is appropriate for severe bleeding, severe pain, or faintness. These symptoms are not explained simply by having an abnormal smear and should be assessed on their own terms.

Questions to bring

What grade of dysplasia was found? Why is excision preferred over monitoring? How much tissue may be removed, and what follow-up will I need?
