When Is Facet Joint Injection Recommended for Facet Joint Syndrome?

Short answer
Facet joint injection is usually considered when facet joint syndrome is suspected after an examination, symptoms persist despite appropriate conservative care, and a clinician believes an injection could help confirm or manage the pain source.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Main purpose
- To help assess or manage pain thought to come from a facet joint.
- Usually considered
- After clinical assessment and when non-invasive care has not been enough.
- Plan afterward
- Continue the clinician's rehabilitation and activity advice when appropriate.
What the recommendation means
Facet joints are small joints at the back of the spine that help guide movement. Irritation in these joints can cause neck or back pain, stiffness, and pain that worsens with extension, twisting, or prolonged positions. Similar symptoms can come from discs, muscles, nerves, or other spinal conditions, so an injection is not selected from symptoms alone.
A clinician may recommend an injection when the pain pattern and physical examination point toward a facet joint, exercises and other non-invasive measures have been tried appropriately, or pain is limiting rehabilitation. The injection may place local anaesthetic, sometimes with an anti-inflammatory medicine, near the suspected joint. Temporary improvement can provide useful information about the pain source, but it does not replace a broader diagnosis or rehabilitation plan.
What to do before and after the decision
Arrange an assessment with a qualified clinician who can review the location and pattern of your pain, movement findings, medicines, allergies, and relevant medical conditions. Bring a list of treatments already tried and describe what made the pain better or worse. Ask whether physiotherapy, activity modification, heat, or other options should continue alongside the procedure.
If an injection is planned, follow the clinic's instructions about medicines and transport. Afterward, follow the advised activity limits and monitor the injection site. A short period of soreness can occur; increasing redness, drainage, fever, new weakness, or worsening pain needs prompt medical advice.
When to seek care urgently
Seek urgent medical care for new loss of bladder or bowel control, numbness around the groin or inner thighs, rapidly worsening leg or arm weakness, or severe pain after a significant injury. These features can indicate a problem that needs prompt assessment rather than an elective injection.
Contact a clinician promptly for fever with back pain, unexplained weight loss, a history of cancer, or pain that is severe at rest or repeatedly wakes you. If you have already had an injection, urgent review is also appropriate for breathing difficulty, facial swelling, widespread rash, or signs of infection at the injection site.
Questions for your doctor
Ask: What findings make you think a facet joint is contributing to my pain? What other causes have been considered? Is the injection intended mainly to confirm the pain source, reduce symptoms, or support physiotherapy? What medicines or conditions could affect the plan? What should I do if it does not help?

Questions people ask
Does needing an injection prove that I have facet joint syndrome?
No. The decision is based on the overall history and examination, and response to an injection may provide information without proving that every symptom comes from the facet joint.
Can physiotherapy still be useful after a facet joint injection?
Often, yes. If pain settles enough to move more comfortably, a clinician may use that window to address mobility, strength, posture, and activity habits safely.
