When Is a Chest X-Ray Recommended for Pneumonia?

Pneumonia can cause changes in the air spaces of the lungs. A chest X-ray may help a clinician see these changes and distinguish pneumonia from other explanations for cough, fever, chest discomfort, or breathlessness. The image is interpreted alongside your symptoms, examination, and medical history; an X-ray alone does not identify every cause of respiratory symptoms.

A clinician may recommend imaging when the examination is unclear, breathing symptoms are significant, or there is concern about a complication. It can also provide a reference when symptoms continue despite treatment.

Tell the imaging team if you are pregnant or could be pregnant, and mention any difficulty standing or taking a deep breath. The test is quick, and you may be asked to stand in different positions while holding your breath briefly. Follow the treatment plan given by your clinician and report worsening breathing, fever, or chest pain rather than waiting for a routine review.

Seek urgent medical help for severe or rapidly worsening breathlessness, bluish lips, confusion, fainting, severe chest pain, or an inability to keep fluids down. A child, older adult, pregnant person, or someone with a long-term heart or lung condition may need prompt assessment when respiratory symptoms appear or worsen.

Ask what finding makes imaging useful, whether the X-ray is needed now, what other tests may be considered, and when you should return if symptoms do not improve.
