When Is Bisphosphonate Therapy Recommended for Hip Fracture?

A hip fracture can signal that the skeleton is less able to withstand an everyday fall or minor force. Bisphosphonates slow the breakdown of bone, giving bone-forming activity more opportunity to maintain strength. The medicine does not repair the fracture itself; surgery, mobility support and rehabilitation address the injury. A clinician usually reviews the fracture history, bone-density assessment, kidney function, calcium and vitamin D status, dental concerns, and other medicines before choosing a drug and route.

Arrange a fracture-prevention review after the acute hip-fracture plan is underway. Bring a list of medicines and supplements, details of previous fractures, and any history of kidney problems or difficulty swallowing. Do not start, stop or delay a bisphosphonate without advice. Your clinician may correct low calcium or vitamin D first and explain how to take an oral medicine with water while remaining upright, or whether an infusion is more suitable. Continue prescribed physiotherapy and use the walking aid recommended for safe movement.

Contact the fracture team promptly for worsening hip pain, a new fall, increasing wound redness or drainage, fever, or a sudden loss of mobility. Seek urgent help for chest pain, severe breathlessness, new confusion, or a painful swollen calf. During treatment, report trouble swallowing, severe heartburn, jaw pain or a new thigh or groin ache so the prescriber can assess it.

Ask whether the fracture suggests osteoporosis, which tests are needed before treatment, and when therapy should begin. Clarify whether an oral or infusion option fits your kidney function and routine, how calcium and vitamin D will be managed, how long the plan may last, and which symptoms should trigger a medication review.
