When is aromatase inhibitor therapy recommended for hormone receptor-positive breast cancer?

Aromatase inhibitors reduce the body’s production of oestrogen, which can slow or stop growth of hormone receptor-positive breast cancer cells. They are commonly considered after menopause; before menopause, ovarian suppression may be needed so the medicine works as intended.

The choice depends on menopausal status, tumour features, lymph-node findings, previous endocrine treatment, other illnesses, bone health, symptoms, and whether treatment follows surgery or is being used for advanced disease. The medicine is usually taken regularly as a tablet.

Tell your team about joint or muscle problems, osteoporosis, fractures, high cholesterol, heart disease, dental issues, and all medicines or supplements. Ask whether you need a bone-density assessment, vitamin and mineral guidance, dental care, or an exercise plan.

Report troublesome stiffness, pain, hot flushes, vaginal or sexual symptoms, mood changes, or unusual fatigue. The team may adjust the medicine, manage symptoms, or discuss another endocrine option; do not stop tablets without speaking with them.

Contact your oncology team promptly for severe or persistent bone pain, a fall with injury, new weakness, swelling or pain in one leg, chest pain, or sudden breathlessness. Seek emergency help for severe breathing difficulty or signs of a serious allergic reaction.

Ask: Am I postmenopausal for treatment purposes? What is the aim and planned duration? How will bone health be monitored? Which symptoms can be treated without stopping therapy? Should I use ovarian suppression or another endocrine medicine? How will response be assessed?
