What to expect from topical retinoids for acanthosis nigricans

Acanthosis nigricans creates darker, thicker, velvety skin, often in folds such as the neck, underarms, groin, or knuckles. A topical retinoid can encourage more orderly shedding of surface cells and may improve texture and colour over time. It does not treat every reason the skin has changed, so improvement may be limited if the underlying metabolic, hormonal, medication-related, or other trigger remains active.

Do not scrub aggressively or use unlabelled lightening products on the area. Ask a clinician to confirm the diagnosis, particularly if the change appeared quickly or is spreading.

Apply a thin amount to clean, completely dry skin as prescribed, avoiding eyes, lips, mucous membranes, and cracked skin. Start only as often as advised; using more can increase irritation without speeding the result. Moisturiser may help, and sun protection is useful for exposed areas because irritation can make discolouration more noticeable.

Burning, peeling, redness, dryness, and tenderness can occur, especially when treatment begins. Contact your prescriber if the reaction is strong or persistent. Discuss pregnancy, plans for pregnancy, breastfeeding, eczema, and other active skin treatments before starting.

Arrange assessment for sudden widespread darkening, severe itch, pain, ulceration, or involvement of unusual areas. Seek review if the skin change does not fit a typical fold pattern or fails to improve after the agreed trial. Stop and seek prompt advice for blistering, marked swelling, or a severe burning reaction.
