Short answer

Topical antibiotic therapy for green nail syndrome is applied as prescribed to a clean, dry nail and surrounding skin, but the green colour may take time to grow out as the nail recovers.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Colour
Existing green pigment grows out rather than vanishing at once.
Moisture
Keep the nail dry and protect it during prolonged water contact.
Red flags
Increasing pain, swelling, drainage, or spreading colour needs review.

Why the nail changes slowly

Green nail syndrome is associated with bacteria in a damp space beneath or beside a nail, especially when the nail has lifted or stays wet. Treatment aims to reduce the bacterial burden and address the moisture that allows it to persist. The discolouration already present cannot be wiped away by medicine; it generally moves outward as the nail grows.

Mild irritation around the nail can occur. A clinician may need to reconsider the diagnosis if the nail becomes very painful, the surrounding skin swells, or several nails change colour.

Protecting the nail

Use the prescribed product on a clean, dry nail and let it dry as directed. Keep the nail short without digging under it, and wear gloves for prolonged contact with water or cleaning products. Dry hands and feet carefully after washing, do not cover the nail with an airtight cosmetic layer, and avoid sharing nail tools. Do not pull off a lifted nail unless a clinician advises it.

When to seek review

Arrange assessment if the green area spreads, drainage appears, pain increases, or the skin becomes red and swollen. Prompt care is appropriate for fever, rapidly worsening inflammation, or significant pain. A nail that remains detached, repeatedly relapses, or does not show a healthy growth boundary should be reviewed, particularly if you have diabetes or reduced immunity.

Questions to ask

Ask how to reach the affected nail space safely, how long to use the product, and whether water exposure or a lifted nail is contributing. Clarify when nail polish can be used again, what healthy regrowth should look like, and whether a sample is needed if colour or pain persists.

Questions people ask

Will the nail look normal immediately?

No. The affected part must usually grow forward before the nail looks clear.

Can I cover it with nail polish?

Ask your clinician; airtight coverage can trap moisture and make monitoring harder.
