What to expect from preconception care for neural tube defects

Neural tube defects develop very early in pregnancy, sometimes before a person knows they are pregnant. The appointment does not predict whether a future pregnancy will be affected. Spina bifida and anencephaly are examples of neural tube defects, but their causes are not always identifiable in one person.

Book a preconception visit before stopping contraception if possible. Bring a list of prescribed medicines, over-the-counter products, supplements, allergies, and previous pregnancy records. Do not stop an essential medicine suddenly; the clinician can weigh its benefits and risks and arrange a safer plan if needed.

Discuss the folic acid dose and when to start it, especially if you have had an affected pregnancy, take a medicine that changes folate metabolism, or have a condition such as diabetes. Work with the clinician on blood-glucose control, balanced nutrition, smoking cessation, and vaccination or infection advice. Once pregnant, arrange maternity care early so the recommended scans can be planned.

Seek advice before conception if you have a previous pregnancy or child with a neural tube defect, a close family history, diabetes, or medicines that may affect folate. After a positive pregnancy test, contact maternity care promptly to confirm supplements and medicines. Urgent assessment is needed for heavy bleeding, severe abdominal pain, fainting, or one-sided pain.
