What to expect from hot and cold therapy for ankle sprain

Short answer

For an ankle sprain, expect cold therapy to calm early swelling and discomfort, while gentle heat may ease stiffness later; neither replaces assessment when symptoms are severe.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Early comfort
Cold is often more suitable while swelling is active.
Later stiffness
Gentle heat may help before movement once swelling has eased.
Skin safety
Use a cloth barrier and stop for numbness or skin colour change.

What the treatment is doing

A sprained ankle has stretched or injured ligaments, often with tenderness and swelling around the joint. A wrapped cold pack can temporarily reduce the feeling of pain and limit activity-related swelling. Heat is more commonly comfortable after the initial swelling has settled; it can make a stiff ankle feel easier to move before gentle exercises. These effects are temporary and do not show whether the ligament has fully healed.

How to use it

Place a cold pack inside a cloth rather than directly on skin, and remove it if the skin becomes numb, pale, or painful. Keep the ankle supported and elevated when resting. Avoid heat over a visibly swollen or freshly injured area, and never sleep with a heating device on. As walking becomes more comfortable, follow a clinician or physiotherapist’s plan for range-of-motion, balance, and strengthening work. Read more about [Ankle Sprain] and [Hot and Cold Therapy].

When to seek care

Seek prompt medical assessment if you cannot take a few steps, the ankle looks deformed, pain is severe, swelling rises quickly, or the foot becomes cold, blue, numb, or unusually weak. Arrange an assessment when symptoms are not steadily improving or when tenderness is directly over a bone, since a fracture can need different care.

Questions for your doctor or physiotherapist

Ask which phase of your injury suits cold or heat, how to protect your skin, and when to begin loading the ankle. Ask how much walking is appropriate and which movement or balance exercises fit your symptoms.

Questions people ask

Can heat make an ankle sprain worse?

Heat may increase discomfort or swelling when used over a fresh, visibly swollen injury, so ask a clinician when to introduce it.

Does cold therapy heal the ligament?

Cold can make symptoms easier to manage, but ligament recovery also depends on protection, progressive movement, and rehabilitation.
