What to expect from a GP consultation for middle ear infection

The GP may ask whether pain followed a cold, whether there is fever, reduced hearing, fluid from the ear, dizziness, or ringing, and how symptoms affect sleep. Using an otoscope, they inspect the ear canal and eardrum for redness, swelling, fluid, or a hole. They may also examine the throat, nose, and nearby lymph nodes because congestion can affect the middle ear.

Findings help distinguish middle ear infection from outer-ear inflammation, trapped fluid, or another cause of pain. Treatment depends on age, symptoms, examination findings, and whether the illness is improving or worsening.

Write down when the pain and hearing change started, any fever, recent colds, allergies, swimming, ear injuries, or discharge. Mention medicines already used and any medicine allergies. Do not put cotton buds, oil, or unprescribed drops into the ear, particularly if fluid is leaking or the eardrum may be damaged. Follow the plan for pain relief and review.

Arrange urgent review for swelling or severe tenderness behind the ear, a facial weakness, severe dizziness, worsening headache, marked drowsiness, or a child who appears very unwell. Prompt assessment is also needed for persistent discharge, sudden hearing loss, or symptoms that are getting worse rather than settling.

Ask what the ear examination showed, whether antibiotics or observation is appropriate, how to control pain, whether water should be kept out, and when hearing or discharge should be checked again.
