What if topical retinoids does not relieve blackheads?

Blackheads form when oil and dead skin cells block a pore and the surface material darkens after contact with air. Ongoing oil production, hair or skin products, friction, and inconsistent use can allow new blockages to appear. Irritation from a retinoid can also cause redness and peeling without removing every existing comedone.

Bumps that look like blackheads may instead be enlarged pores, sebaceous filaments, or another type of acne. Correct identification helps avoid harsh squeezing and unnecessary combinations.

Use the retinoid exactly as prescribed, apply it to clean dry skin as directed, and avoid adding scrubs or several exfoliating products at once. Choose non-comedogenic moisturiser and protect exposed skin from sunlight. Do not squeeze blackheads, because pressure can worsen inflammation and leave marks. Note the areas affected, irritation, and products used so a clinician can refine the plan.

Arrange a review if blackheads persist despite correct use, spread, or are joined by painful lumps, scarring, or significant irritation. Seek prompt advice for swelling around the eyes, severe skin reaction, or widespread blistering. A clinician can check whether the lesions are comedonal acne and whether another treatment is needed.
