Why fatigue may persist

Counselling can help with distress, coping and sleep routines, but fatigue may also reflect anaemia, infection, thyroid problems, pain, dehydration, poor nutrition, medication effects, treatment changes or reduced activity. Low mood and disrupted sleep can add to physical tiredness without being the only cause. The team may ask how fatigue affects walking, concentration, washing and eating, then review medicines and arrange suitable tests. Persistent fatigue is not a sign that you have failed to cope.

Making the next review useful

Keep a short record of energy levels, sleep, food and fluids, activity, symptoms and medicine timing. Plan essential tasks for the part of the day when you have more energy, sit for tasks when safe, and use help with meals or travel. Gentle movement may be useful if your team says it is safe, but rest between activities rather than pushing through exhaustion. Continue counselling if it supports you, while asking for a medical fatigue assessment.

When to seek urgent help

Contact the team urgently for sudden severe weakness, breathlessness at rest, chest pain, fainting, confusion, a high temperature, new one-sided weakness or inability to stay awake. Seek prompt review when fatigue is rapidly worsening, prevents basic self-care, or comes with bleeding, persistent vomiting or very little urine. These changes can need medical treatment rather than a counselling adjustment alone.

Questions for your doctor

Ask which reversible causes should be checked, whether any medicine or treatment could be contributing, and what activity level is safe. Ask for help with sleep, nutrition and practical support, and agree on a contact point if fatigue suddenly changes.
