What if hormone replacement therapy does not relieve night sweats?

Short answer
If hormone replacement therapy does not relieve night sweats, ask your prescriber to review treatment, triggers, sleep disruption and other possible causes.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Persistent night sweats deserve a treatment and health review.
Why sweating may continue
Night sweats can persist when HRT needs adjustment, doses are missed, or the delivery method is not suited to you. A warm room, alcohol, spicy food, stress, some medicines, infection or thyroid problems can also cause sweating. Not every night sweat is caused by menopause.
Record whether you wake with damp clothes or bedding, feel hot or chilled, have palpitations, lose weight, develop a cough or feel unwell. Your clinician will use this pattern, your medical history and your medicines to decide what assessment is needed.
Making nights easier
Keep the bedroom cool, use breathable layers and place spare sleepwear nearby. Limit personal triggers that clearly precede episodes and keep a consistent sleep routine. A symptom diary can show whether sweating follows a dose, meal, stressful event or room-temperature change.
Arrange a treatment review rather than increasing HRT yourself. Ask whether the timing, dose or form should change and whether a non-hormonal option or investigation is suitable. Mention unexplained bleeding, breast changes, migraines, clotting history and every supplement you use.
When to seek medical help
Seek prompt care for night sweats with fever, persistent cough, unexplained weight loss, swollen glands, severe weakness or feeling acutely unwell. Get urgent help for chest pain, difficulty breathing, fainting, sudden weakness or confusion. New unexplained vaginal bleeding during HRT also needs medical review.
Questions for your appointment
Ask whether the sweating pattern fits menopause, how long a treatment change should be assessed, which medicines can cause sweating, and what warning symptoms should bring you back sooner.

Questions people ask
Can I take an extra HRT dose at night?
Do not take extra hormones without prescriber advice. The timing and dose should be checked against your treatment and health history.
When are night sweats less typical of menopause?
