What if emollient therapy does not relieve cracked skin?

Short answer
If emollient therapy does not relieve cracked skin, protect the area and arrange a clinical assessment for inflammation, infection, or another cause needing targeted treatment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What persistent cracks can indicate
Cracking means the skin barrier is breaking down. Repeated hand washing, detergents, cold or dry air, friction, and work involving water can prevent the split from closing. Eczema, contact dermatitis, psoriasis, or a fungal infection may also cause scaling and fissures, so moisturising alone may not address the driver. Cracks around the fingers, heels, or other high-use areas can reopen with movement.
How to protect the area
Apply a suitable fragrance-free emollient after washing and whenever the skin feels tight, following product directions. Wash with lukewarm water, pat dry, and avoid picking loose edges. Use protective gloves for wet or irritating tasks, with a soft cotton layer if sweating is a problem. Do not put strong antiseptics or steroid products into an open fissure unless a clinician has advised you. Take a photograph and note exposures if the cracks keep returning.
Warning signs
Obtain prompt care for increasing pain, warmth, swelling, pus, red streaks, fever, or rapidly spreading redness. Seek assessment when a crack is deep, repeatedly bleeds, limits movement, or fails to close despite protection. See [Cracked Skin](/symptoms/skin-cracking/) for related symptom guidance.
Questions for your appointment
Ask whether the cracks suggest eczema, contact dermatitis, infection, or another problem; whether a dressing or prescribed medicine is needed; and which ingredients or workplace exposures to avoid. Ask how to use [Emollient Therapy](/treatments/medical-dermatology/emollient-therapy/) around open skin.

Questions people ask
Can I cover cracked skin?
A suitable protective dressing may reduce friction, but deep, wet, or infected-looking cracks should be assessed before self-treatment.
Why do cracks reopen after moisturising?
Movement, repeated exposure, inflammation, or an untreated infection can continue stressing the skin barrier.
