What if dry needling does not relieve neck stiffness?

Dry needling is aimed at selected tight or sensitive muscles. Neck stiffness may instead be linked to a joint, sustained screen posture, a recent strain, headache-related muscle tension, or irritation of a nerve. The technique may have reduced one tight spot without changing the movement or load that keeps symptoms active. Describe whether stiffness is one-sided, whether it restricts turning, and whether pain travels into the shoulder or arm.

Use gentle neck movement within a comfortable range and take regular breaks from prolonged screen or driving positions. Follow the specific advice given after treatment, and do not force stretches or repeatedly crack the neck. A physiotherapy review may add posture changes, mobility work, strengthening, manual treatment, or a graded return to normal activity. Bring up headaches, arm symptoms, or dizziness before further needling so the plan can be adjusted safely.

Seek urgent care for neck stiffness with fever, a severe unusual headache, confusion, fainting, new arm or leg weakness, trouble walking, or loss of bladder or bowel control. Get prompt assessment after a significant injury or when pain, numbness, or weakness is spreading.

Ask what is limiting neck movement, whether the symptoms fit a muscle problem, which exercises are appropriate, and what changes would mean that needling should be postponed.
