What If Cortisone Injection Does Not Relieve Joint Swelling?

Swelling can reflect inflammation, fluid inside the joint, a recent injury, bleeding, infection, crystal disease, or irritation around the joint. Cortisone may not address every cause, and swelling can also come from a nearby bursa or tendon rather than the joint itself. Describe whether the swelling is hot, red, soft, tense, or linked to movement. Mention fever, a wound, blood-thinning medicine, and any injury so the clinician can judge urgency.

Follow the injection aftercare advice and avoid heavy loading of the affected joint until you know why it remains swollen. Elevate the limb when comfortable, use only the recommended cold or pain measures, and do not massage a hot or markedly tender area. Note the swelling's change, joint movement, skin appearance, and ability to bear weight. A clinician may consider examination, fluid testing, imaging, or a different treatment depending on the findings.

Seek urgent medical care for rapidly increasing swelling, severe pain, a hot red joint, fever, chills, pus or drainage, or feeling suddenly unwell after the injection. Get prompt assessment if you cannot move or bear weight through the joint, or if the limb becomes numb, pale, or unusually cold. Infection and other urgent causes need timely treatment.

Ask whether the swelling is inside the joint, whether fluid should be tested, which activities are safe, how to monitor the injection site, and which changes require immediate care.
