What happens if vertebral compression fracture is not treated?

A compression fracture occurs when a spinal bone partly collapses. Without assessment, the collapse may heal in a wedge-shaped position. This can increase a forward curve in the upper back, alter posture and make standing or walking tiring. Persistent pain can also lead to guarded movement, muscle weakness and difficulty with everyday tasks.

One fracture may signal reduced bone strength, including osteoporosis, so ignoring it can leave the underlying cause unaddressed. New fractures can occur in nearby vertebrae, especially when the spine is carrying uneven loads.

Arrange a clinical assessment rather than relying on rest alone. A clinician may review the injury, examine your nerves and request imaging to confirm the fracture and its age. Treatment can include activity guidance, pain relief, a brace in selected cases, and a plan to protect bone strength. Gradual rehabilitation helps restore safe movement and trunk strength. Bisphosphonate therapy may be discussed when osteoporosis contributes to fracture risk; procedures such as kyphoplasty or vertebroplasty are considered only for selected patients with persistent symptoms.

Seek urgent medical help for new leg weakness, numbness spreading down a leg, loss of bladder or bowel control, inability to walk, or severe pain after a fall. These symptoms can indicate nerve involvement or a more serious injury. Arrange prompt review for sudden new upper or lower back pain, marked height or posture change, or pain that prevents sleep or basic movement.

Ask whether imaging is needed, whether your spinal nerves are affected, and which movements or lifting tasks to avoid. You can also ask how bone strength will be checked, when rehabilitation should begin, and why a brace or a spinal procedure might or might not suit your symptoms.
