What happens if myelofibrosis is not treated?

Short answer
If myelofibrosis is not treated, scarring in the bone marrow may worsen, blood counts may fall, and the spleen may enlarge and cause increasing symptoms.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Core problem
- Marrow scarring that can reduce healthy blood-cell production
- Common physical effect
- An enlarged spleen causing fullness or abdominal discomfort
- Urgent signs
- Heavy bleeding, severe pain, fainting, or high fever
What progression can involve
Myelofibrosis changes the bone marrow environment and can make blood-cell production less effective. Low red-cell counts may cause tiredness, weakness, or breathlessness. Low platelets can increase bruising or bleeding, while abnormal white-cell production can affect infection defence.
The spleen may take on more blood-making work and become enlarged, causing Abdominal Swelling, fullness after small meals, or discomfort under the ribs. Some people also develop Night Sweats, fever, bone discomfort, or unintended weight loss. The pattern and speed of change vary.
What to do after diagnosis
Arrange regular review with a haematology team. Blood counts, examination, symptom history, and sometimes marrow or imaging tests help show whether the condition is stable or changing. Tell the team about increasing abdominal fullness, fever, bleeding, or reduced stamina.
Treatment may include Targeted Therapy to control symptoms and blood counts. Allogeneic Stem Cell Transplant may be discussed for selected people because it carries substantial preparation and follow-up needs; suitability depends on health, disease features, and donor availability.
When to seek urgent care
Seek urgent help for severe abdominal pain, fainting, heavy bleeding, black stools, chest pain, severe breathlessness, or a high fever with shaking chills. A rapidly enlarging abdomen or sudden marked weakness also warrants prompt assessment.
Questions for your doctor
Which blood counts are affected? Is my spleen enlarged, and what symptoms should I watch for? What is the aim of targeted treatment? Could transplant be suitable, and what preparation would it require? How often should I be reviewed?
Questions people ask
Why can myelofibrosis cause abdominal swelling?
The spleen may enlarge as it takes on blood-making work, leading to fullness, discomfort, or visible abdominal swelling.
Does everyone with myelofibrosis need a transplant?
No. Transplant is considered selectively after weighing disease features, general health, risks, donor options, and personal goals.
